Scholars Programs Inc. is dedicated to supporting youth and families in overcoming significant challenges. Our mission is to empower those dealing with Serious Emotional Disturbance (SED) through comprehensive support and services. We are searching for empathetic and dedicated individuals to enhance our team as CMHW Medicaid providers. This role is crucial for supporting youth with Serious Emotional Disturbance (SED) and their families. We are hiring for two positions: FST and Habilitation.

Open Positions

Family Support Specialist

We are seeking a compassionate and dedicated Family Support Training (FST) provider to join our team. In this role, you will work closely with families, providing crucial training and support to enhance their ability to support their SED-affected youth effectively. The intent of the service is to provide education and support to the caregiver, which preserves the family unit and increases confidence, stamina, and empowerment. Training and support activities, and the providers selected for these activities, are based on the family/caregiver’s unique needs.

Key Responsibilities:

  • Delivering tailored training sessions to families, focusing on strategies and skills to support their children.

  • Collaborating with a multidisciplinary team to create and implement individualized support plans.

  • Monitoring and reporting on the progress of families and youth under your care.

  • Maintain documentation requirements.

Position Type: 1099 Contractor
Pay: $35/hour

Habilitation Specialist

As a Habilitation provider, you will play a vital role in delivering specialized services designed to foster the development of life skills in youth with SED. This position requires a compassionate approach and a commitment to enhancing the well-being of the individuals we serve. Habilitation services are to enhance the participant’s level of functioning, quality of life, and use of social skills, as well as build the participant’s and family’s strengths, resilience, and positive outcomes. The Habilitation service provider helps the participant accomplish these goals through the development of the following skills:

  • Identification of feelings

  • Managing anger and emotions

  • Giving and receiving feedback, criticism, or praise

  • Problem-solving and decision-making

  • Learning to resist negative peer pressure and develop prosocial peer interactions

  • Improving communication skills

  • Building and promoting positive coping skills

  • Learning how to have positive interactions with peers and adults

Key Responsibilities:

  • Implementing habilitation services according to the Plan of Care (POC)  tailored to the individual needs of SED youth.

  • Working closely with families and other team members to promote the integration of habilitation strategies into daily life.

  • Assessing and documenting the progress of youth, adjusting plans as necessary to meet their evolving needs.

  • Maintain documentation requirements.

Position Type: 1099 Contractor
Pay: $45/hour

Qualifications:

  • High School Diploma or equivalent.

  • Minimum 2 years of experience working with SED youth and their families.

  • Pass background checks and drug screen.

  • Satisfactory driving history.

  • Effectively operate a computer and typing skills.

  • Strong commitment to making a positive impact.
